Nursing Assistant Med Tech

The Med Tech is responsible for coordinating and managing client medical appointments while supporting nursing staff with medication-related tasks, durable medical equipment coordination, and documentation. This role ensures continuity of care, compliance with healthcare requirements, accurate Therap documentation, and organization of medication and medical supply systems within the home.

Responsibilities:

Appointment Scheduling & Coordination (Medical, Dental, Specialty, Follow up)
  • Ensure appointments are arranged in a timely manner based on required frequency and provider recommendations
  • Communicate appointment details with program staff, nursing, and relevant team members
  • Reserve company vehicles for all scheduled appointments requiring transportation
  • Accurately enter all scheduled appointments into Therap
  • Input all scheduled labs, imaging, diagnostics, or testing into Therap once scheduled to ensure visibility and tracking
  • Regularly review Therap to ensure each individual remains current on required appointments
  • Monitor upcoming, overdue, and annual healthcare needs proactively
  • Review patient portals for provider documentation
  • Contact the clinic/provider directly if additional records are needed
Medication Support, Delivery, & Pharmacy Responsibilities
  • Assist the RN with medication management tasks as delegated
  • Assist with implementing medication changes following provider appointments and RN direction
  • Maintain cleanliness, organization, and tidiness of medication cabinets, medication rooms, and medication storage areas
  • Monitor medication supplies and communicate refill needs to nursing staff as appropriate
  • Report any missed medications, medication concerns, discrepancies, or errors to the RN promptly
  • Pick up and deliver medications from the pharmacy and/or designated medication delivery locations
  • Ensure medication delivery sheets are signed and dated appropriately
  • Assist with picking up off-cycle medications from local pharmacies, including urgent medications such as antibiotics
  • Communicate all new medication orders, medication changes, and urgent pharmacy updates with the program nurse and program manager promptly
  • Assist with ordering durable medical equipment (DME) supplies as directed
  • Pick up and return used oxygen tanks as needed.
Annual Physicals, Dental Care, Labs, Imaging, & Diagnostic Supports
  • Ensure every individual receives a yearly physical assessment and follow up on missing or expired physicals and PSOs
  • Schedule dental cleanings every 6 months unless otherwise directed by the dentist
  • Ensure all labs, imaging, and diagnostic testing are entered into Therap once scheduled
  • Track completion and follow up on missing results or documentation as needed

Requirements:

  • Experience with caretaking, nursing, CNA, and/or experience with developmental disabilities is preferred
  • At least 21-yrs-old with at least 3 years of driving experience and valid driver's license to help facilitate resident appointments
  • Must be able to lift up to 50 pounds
